Designing a home-grown digital laboratory ecosystem: a laboratory professional initiative to improve safety, efficiency, and sustainability

Abstract
Introduction: Clinical laboratories often struggle with paper-based workflows, fragmented communication, and limited specimen visibility. At New Mowasat Hospital, Kuwait, manual transcription, printed worksheets, and difficulty tracking pending or add-on tests contributed to delays, inefficiencies, and patient safety risks. Laboratory professionals collaborated with the IT Department to develop a fully home-grown digital laboratory ecosystem tailored to local operational requirements.
Objective: To implement a sustainable digital laboratory workflow that improves traceability, turnaround time, communication efficiency, operational resilience, patient safety, and environmental sustainability while reducing financial burden through in-house innovation.
Results: Beginning in July 2021, a phased digital transformation program replaced manual registers and worksheet-based processes with real-time electronic solutions. An electronic Specimen Collection and Receiving Log enabled end-to-end sample traceability and reduced transcription-related errors. An internally developed Electronic Result Indexer consolidated analyzer outputs into prioritized workflows for ER/ICU samples, critical alerts, and high-risk analytes. Additional features included automated flagging, second-opinion routing, status-based filtering, and integrated communication tools. The initiative achieved 100% compliance in critical alert communication, improved turnaround times, enhanced workflow transparency, and reduced paper consumption by over 80%. Internal development avoided an estimated capital expenditure of USD 113,000 and recurring annual costs of USD 10,000. The initiative strengthened collaboration among laboratory, nursing, and phlebotomy teams.
Conclusion: A laboratory professional-led, home-grown digital ecosystem can improve efficiency, sustainability, communication, workflow resilience, and patient safety while delivering measurable financial savings and scalable future-ready innovation.
